How To Request Un-Banning Yourself?

Hi all, what's up?!!
In order to request to un-ban yourself form our server, you should post a new topic here mentioning the following:

Nickname: Your server name.
Time/Date: Time/date of ban (if you don't know try to remember and guess).
Reason: The reason you were banned (it is displayed to you on your screen when you are banned).
IP Used: The IP address you used when you got banned.
Admin Name: The name of the admin that banned you.
Extra Notes: Any additional information you feel is relevant to the ban appeal.
Screen Shot: If possible, take a screen shot of the banning.

After writing the topic, the forum and server main administrator will reply to your topic as soon as possible to tell you whether he accepted your appeal or not.
